Why Has Male Fertility Been Overlooked?

  • History & Culture: Fertility medicine developed with a focus on women, and society has long assumed infertility is “her problem.”

  • Testing Differences: Male testing is relatively simple (a semen analysis), whereas female testing is complex and invasive, leading doctors to focus there.

  • Treatment Bias: Many fertility treatments—like IVF and egg retrieval—are built around the female body, reinforcing the imbalance.

  • Assumptions About Technology: The use of ICSI (injecting sperm directly into the egg) has led to a belief that male factors can simply be bypassed.

Subfertility vs. Infertility

It’s important to make a distinction:

  • Subfertile: Couples who don’t conceive in the first 6 months. Many of these couples go on to conceive naturally in the next 3 years.

  • Infertile: Couples still not pregnant after 4 years. At this stage, natural conception is highly unlikely.

  • Sperm counts have fallen by nearly 30% in just over a decade (1989–2002).

  • Both sperm quality and quantity continue to decline worldwide, with regional variations.
